Education requires the sharing of experiences. For this reason, The 12th CTO Club is making a Call for Cases. Physicians and fellows may submit one of their own clinical cases from the field of CTO.
Each case submitted will be assessed by our referees and a successful applicant will be invited to present their cases during Case Review Sessions at The 11th CTO Club.The best submission will be awarded recognition and invited to be a faculty member of the 13th CTO Club.

- After sending the form, registered number will be issued and also sent by e-mail.
(Please keep the registered number.) - Print out the confirmation screen and send it with CD-ROM to CTO Club Secretariat by courier service.

[3] Acceptance/Rejection Announcement & Excellent Cases
- Only English can be used.
- Exemplary cases will be selected by referees among accepted and the selected presenters will be invited to give presentations at the Case Review Sessions, using PC projecter.
- The 11th CTO Club Course Directors reserve the right to make final decision on Acceptance/Rejection of cases. The results will be posted on CTO Club website and notified by e-mail in the begining of May.

- The total number of words is not limited.
- Be arranged in the following "Target Lesion", "Strategy" and "Final Result".
- Using table or chart is not acceptable.
